You may notice that sometimes there's a plot thread that looks like it can be taken further but we end up never hearing of it again. Here are a couple I have noticed. Keep in mind that I've only seen the first three seasons, so if one of these reappears in seasons four or five, then I apologize.
1. The vicker recording from Deathwalker. Other than a brief mention in Divided Loyalties, this one disappears from the face of the universe. Not surprising, since I seem to recall reading somewhere that JMS didn't like the concept of vickers.
2. Bureau 13 in A Spider in the Web. After this mention, nothing. Probably just as well, as the show seemed to be getting a bit overloaded with conspiracies.
Anything else?
The Vicker recording went nowhere because Andrea Thompson left the show and went onto NYPD Blue. The origional plan was for them to restore her personality and everything that later happened to Lyta Alexander to happen to Talia instead. When Thompson left JMS asked Patricia Talliman to return and take her place.
The Bureau 13 thread went nowhere because JMS found out there was an RPG by the same name and decided out of courtesy to drop the name.
Did we ever really find out what the true, original purpose of the Great Machine was?

Now I don't know where I read this from, but I do remember hearing that if Sinclair had remained in the show, his transformation to Valen would have been the finale, thus the great machine serves its purpose in the show. But as he did not the great machine pretty much sunk after "war without end.".
Keeping in mind that I've only seen the first three seasons, did they ever contact the Sigma 957 First Ones after Voices of Authority?
They did in season 4.
I think the issue of how the (would-be) assassin in the pilot was able to operate so effectively was dropped (because the actress playing Laurel Takeshima left the show).
Jason Ironheart was, indeed, never seen again after "Mind War".
The Narn from "All Alone in the Night" later visited the station, but I don't remember anything coming of that.
There were hints of "other mistakes" by the Vorlons in Thirdspace, but those were never developed.
I guess the whole story of Crusade was unresolved (unless in books).

That Narn's name was Ta'Lon played by Marshall Teague, and I believe he had quite a few appearances.
The other mistakes by the Vorlons...well considering how ancient they are, some of these mistakes like the Thirdspace artifact might be ancient. Also, maybe Lyta is implying that the Vorlons and the Shadows whole philosophy was flawed in the first place.

The alien probe that showed up and 'tested' them. The aliens that abducted Sheridan. What was the deal with the other Vorlon shot in "Deathwalker?"
I figgured the whole "so many mistakes" line was added to leave a door open just in case some came up with a really interesting idea somewhere down the line("Oh, let's say that was the Vorlons. That'll work.")
Ah, the aliens that abducted Sheridan (early in the second season if I remember correctly), were those not Drakh?
Nope, not the Drakh... can't remember the name of the species offhand, though.

The other Vorlon shot during Deathwalker was actually a Nakaleen feeder. A mindenergy vampire from the Centauri world of Nakaleen.

JMS didn't like the idea of God-like individuals walking around his universe. All his characters were human (or Narn, Centari, Mimbari ect) so Iron Heart was never coming back.
The unresolved plot point (more or less) was the 'gift' he gave to Talia - her increased powers. This was checked in later series twice - once after Talia was able to wipe Besters mind, and later when Bester used her name to try and get Garibaldi angry.
Taila's gift was going to be the dramatic increase in her powers that would have allowed her do all of those things that Lyta Alexander went on to do, which was explained by her having been altered by the Vorlons.
