All right, here goes:

White moves the pawn in front of his kingside bishop one square. Black moves the pawn in front of his king two squares. White moves the pawn in front of his kingside knight two squares. Black moves his queen all the way diaginally and checkmate. Admittedly, it requires White to make a couple of really idiotic moves.

Well, the damaged Damage Report Machine has an excuse for making a mistake in the summary. Queeg'a face appears to them in the Hologram Simulation Room. I don't think we even see the Drive Room in this episode.
Personally my favorite quote, of all the wonderful lines in this episode, has to "April, May, June, July, & August Fools!" - Holly. I'm just waiting for a chance to use that in real life.

Holly says a "meteor" has hit the ship. It is only a meteor when it is in the atmosphere of a planet, in space it is a meteoroid. (Or an asteroid, but that term is usually used when it is larger than a kilometer.)

In the remastered version they added some visual static to Rimmer when he's having his personality problems.

So did Holly engineer the problems with the Hologram Simulator or did a meteorite really hit the ship and Holly decided that was the right time to spring Queeg on the crew?

Yes, it's His "Practical Joke" on Lister, Rimmer & The Cat, as Holly feels that the crew aren't appreciating "Him" enough ! Personally, I believe that he "Engineered" the Hologram Simulator "Problems" as part of the Joke.
